Job summary

Heart to Heart Hospice in Houston is seeking a Social Worker to assess patients and identify psychosocial, grief, financial, environmental and community needs with referrals to resources.

You will provide individual counseling, crisis intervention, advance directives assistance, and support for funeral planning and legal/fiscal transitions. On-call coverage, timely documentation, and compliance with laws and policies are required.

Qualifications

• Bachelors or Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• One year of experience as a Social Worker in a clinical care setting preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education andcompetence to meet the needs of the patients
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
