Foothill Acres Nursing & Rehab Center is seeking a full-time Social Worker for our 200-bed LTC/SAR facility. Primary duties include participating in care plan meetings, coordinating discharge planning, and conducting MDS assessments to support patient well-being.

The ideal candidate is caring, compassionate, and able to work within an interdisciplinary team. LSW certification and at least one year of SAR/Long Term Care experience are preferred.

Qualifications

• LSW preferred or equivalent social work license.
• At least 1 year of SAR/Long Term Care experience preferred.

Responsabilités

• Attend care plan meetings and coordinate discharge planning.
• Conduct MDS assessments.
• Address patients' psychosocial needs as part of an interdisciplinary team.

The ideal candidate will be caring and compassionate and able to work as part of an interdisciplinary team. LSW and one year experience in a SAR/Long Term Care setting preferred.
