Job summary

The Oxford Rehabilitation & Health Care Center in Haverhill is seeking a part-time social worker to plan, develop, and direct social services for residents at a 120-bed facility. Shift is 8a-4p with flexibility on days and weekly pay between $35–$38 per hour.

Required are a MA social work license (LSW/LICSW/LSWA/LCSW), a bachelor's degree in social work, and at least 1 year of related experience in a hospital or skilled facility.

Qualifications

• Must possess a Massachusetts social work license (LSW, LICSW, LSWA, or LCSW).
• Minimum of 1 year social work experience in hospital, skilled facility, or health-related field preferred.
• Minimum requirement: bachelor's degree in social work from an approved program.

Responsibilities

• Coordinate and monitor resident care conferences.
• Work with residents, families, and responsible parties toward adjustment to placement and crisis.
• Protect residents' health information in accordance with privacy policies.
• Complete comprehensive documentation in a timely manner.
• Advocate for residents' personal needs and coordinate resources.
• Utilize community resources appropriately.
• Support staff on resident-related issues.

The Oxford Rehabilitation & Health Care Center in Haverhill is hiring a part-time social worker!

Shift is 8a-4p, flexible on days, with weekly pay ranging from $35 to $38 an hour. We are a 120-bed skilled nursing facility with a large SUDS population.

As a Social Worker, your role is to plan, organize, develop, and direct all required social services to meet the needs of residents to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Experience & Education:

• Must possess a social work license in the state of Massachusetts (LSW, LICSW, LSWA, and/or LCSW).
• A minimum of one (1) year social work in a hospital, skilled facility, or other health-related field preferred.
• Must possess, as a minimum, a bachelor's in social work from an approved school of social work.

Duties & Responsibilities:

• Coordinate and monitor resident care conferences.
• Work with individual residents, responsible parties, and families towards adjustment to placement and to crisis (i.e., illness, disability, and death).
• Complies with facility Privacy Practices policies and procedures and protects residents’ individual health information.
• Complete comprehensive documentation in a timely manner.
• In the role of advocate coordinate personal needs.
• Utilize community resources appropriately.
• Support staff in relation to resident-related issues.
• Other duties deemed appropriate and assigned by your supervisor.
