Job summary

Northwood Rehabilitation & Health Care Center in Lowell, MA is hiring a Social Worker for 32 hours per week, 8:30 am–4:30 pm. Weekly pay ranges from $34 to $38 per hour, depending on license and experience.

The role plans, organizes, and directs social services to meet residents' emotional and social needs, coordinates care conferences, supports families, maintains privacy, and documents assessments promptly.

Qualifications

• Must hold an active MA social work license (LSW/LICSW/LSWA/LCSW).
• Minimum 1 year social work in hospital, skilled facility or health-related field preferred.
• Bachelor's degree in social work from an approved school.

Responsibilities

• Coordinate and monitor resident care conferences.
• Work with residents, families and parties toward placement adjustment and crisis management.
• Protect residents' privacy and health information following policies.
• Complete comprehensive documentation in a timely manner.
• Advocate for residents and coordinate personal needs with community resources.
• Support staff on resident-related issues.
• Perform other duties as assigned by supervisor.

Job description

We are hiring a Social Worker to join our dynamic care team at Northwood Rehabilitation & Health Care Center in Lowell!

32 hours a week on 8:30a-4:30p. Weekly pay ranges from $34 to $38 an hour, depending on license and experience.

As a Social Worker, your role is to plan, organize, develop, and direct all required social services to meet the needs of residents to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Experience & Education:

• Must possess a social work license in the state of Massachusetts (LSW, LICSW, LSWA, and/or LCSW).
• A minimum of one (1) year social work in a hospital, skilled facility, or other health-related field preferred.
• Must possess, as a minimum, a bachelor's in social work from an approved school of social work.

Duties & Responsibilities:

• Coordinate and monitor resident care conferences.
• Work with individual residents, responsible parties, and families towards adjustment to placement and to crisis (i.e., illness, disability, and death).
• Complies with facility Privacy Practices policies and procedures and protects residents’ individual health information.
• Complete comprehensive documentation in a timely manner.
• In the role of advocate coordinate personal needs.
• Utilize community resources appropriately.
• Support staff in relation to resident-related issues.
• Other duties deemed appropriate and assigned by your supervisor.
