Job summary

Green Key Resources is seeking a Licensed Clinical Social Worker (LCSW) for an outpatient behavioral health position on Long Island, NY. The LCSW will conduct psychosocial assessments, provide individual and couples therapy, and collaborate with psychiatrists and primary care teams to ensure integrated care.

Qualifications

• MSW from an accredited U.S. program.
• Active NYS LCSW license required.

A well-established healthcare organization is seeking Licensed Clinical Social Workers (LCSWs) to join its behavioral health team. There is one opening at each location, offering an opportunity to provide meaningful clinical care within a collaborative, patient-centered outpatient environment.

Position Overview

The LCSW will provide comprehensive behavioral health services to patients and families, including psychosocial assessments, individual and couples therapy, treatment planning, crisis intervention, and behavioral health consultation.

This position works closely with psychiatrists, primary care providers, and other members of the care team to ensure patients receive coordinated, high-quality care.

Key Responsibilities

• Conduct comprehensive psychosocial and clinical assessments
• Develop diagnoses utilizing DSM-5 and ICD-10 guidelines
• Develop, implement, and regularly update individualized treatment plans
• Provide individual and couples psychotherapy
• Provide counseling, psychoeducation, and behavioral health interventions
• Provide crisis intervention and assist with safety planning when needed
• Collaborate closely with psychiatrists, primary care providers, and other healthcare professionals
• Make appropriate referrals and coordinate additional services
• Participate in multidisciplinary team meetings and case conferences
• Complete assessments, treatment plans, progress notes, discharge documentation, and other required documentation
• Maintain accurate and timely documentation within the EMR
• Participate in ongoing clinical supervision, in-service education, and quality improvement initiatives
• Educate patients about available programs and supportive services
• Maintain compliance with NYS licensing and continuing education requirements
• Travel between agency locations as needed

Qualifications

• Master's Degree in Social Work from an accredited U.S. program
