Job summary

Green Key Resources is seeking an experienced Licensed Clinical Social Worker (LCSW) to join its Behavioral Health team in New York City.

In an integrated primary care setting, the LCSW will perform psychosocial assessments, psychotherapy, treatment planning, crisis intervention, care coordination, and collaboration with a multidisciplinary team.

This full-time union position requires NYS licensure and a Master's degree in Social Work or a related field.

Qualifications

• Active NYS LCSW, LMHC, or LMFT license required.
• Master's or advanced degree in Social Work, Mental Health Counseling, or related field.
• Experience providing behavioral health services in a clinical setting.
• Experience with electronic medical records and clinical documentation.
• Spanish or an Asian language is a plus.
• Experience with underserved or multicultural populations preferred.
• Experience with eClinicalWorks (eCW) preferred.

Responsibilities

• Conduct psychosocial and diagnostic assessments using DSM-5 and ICD-10 criteria.
• Develop and implement individualized treatment plans.
• Provide individual, family, and group psychotherapy.
• Provide counseling, psychoeducation, and behavioral health interventions.
• Provide crisis intervention and support for patients experiencing behavioral health emergencies.
• Collaborate with primary care providers and make appropriate referrals.
• Participate in multidisciplinary team meetings and case conferences.
• Maintain accurate and timely documentation in the EMR.
• Meet established clinical utilization and billing targets.
• Participate in Quality Improvement and Quality Assurance initiatives.
• Contribute to behavioral health policies, procedures, and program development.
• Maintain compliance with NYS Department of Health requirements and applicable standards of care.
• Support patients in understanding and participating in their own treatment plans.

Position Overview

The LCSW will provide comprehensive behavioral health services to patients and families of all ages within an integrated primary care setting. • Ability to build rapport quickly and work with patients from diverse backgrounds
• Strong organizational and documentation skills
• Ability to work independently as well as collaboratively within a multidisciplinary team
• Experience working with underserved or multicultural populations strongly preferred
• Experience with individuals with serious mental illness, SUD, HIV/AIDS, chronic illness, LGBTQ+ populations, immigrants, or developmental disabilities is a plus
• Spanish or an Asian language is a plus
• Experience with eClinicalWorks (eCW) preferred
