Join our team at Avalon Rehabilitation and Healthcare Center as a licensed Social Worker. This full-time role offers a competitive rate range of $25/hr to $40/hr based on experience, and requires an NJ SW License or CSW.

Qualifications

• Must have SNF/LTC experience.
• Strong knowledge of regulations governing nursing care facilities.
• Experience in a nursing home or rehabilitation center strongly preferred.

Responsibilities

• Evaluate current, new, and prospective residents to assess needs.
• Address resident concerns and maintain ongoing communication.
• Build and maintain positive relationships with residents and families.
• Coordinate care with interdisciplinary teams.
• Develop and update individualized psychosocial care plans.
• Provide counseling and emotional support as needed.
• Ensure compliance with facility policies and documentation standards.
