Job summary

Ascension Living is seeking a Social Worker in Flint, MI to conduct assessments, coordinate treatment plans with a diverse care team, and connect participants and caregivers with community resources. This full-time role emphasizes interdisciplinary collaboration and proactive discharge planning within the PACE Michigan program.

Required is a Master’s in Social Work and Michigan licensure, with a preference for 3 years of case management experience.

Qualifications

• Master's degree in Social Work is required.
• Michigan licensure (limited or full) is preferred/required.
• 3 years of experience in case management or related field preferred.

Responsibilities

• Conduct interviews and assessments to establish participant priorities and ensure accurate case documentation.
• Coordinate treatment plans with interdisciplinary teams, agencies, and caregivers to ensure compliance.
• Facilitate referrals to community resources and support programs for participants and caregivers.
• Build strong community partnerships to address participants' evolving health and social needs.
• Manage concurrent reviews and discharge planning, coordinating social services and post-discharge support.

What Minimum Qualifications You'll Need

Licensure / Certification / Registration:

Preferred Credential(s):

• Drivers License.
• Masters Level Social Worker.

Education

• Master's degree in Social Work required.

Work Experience

• 3 years of experience preferred.

What Additional Preferences You'll Need

• 1-2 years of experience in case management, home care and/or clinical therapy required.
• Limited or full licensure in the State of Michigan is required.
