Job summary

Angels Care Home Health is seeking a licensed MSW to deliver coordinated in‑home care across all ages. You will perform psychosocial assessments, provide counseling, and connect patients and families with needed resources to support care plans in Flagstaff, AZ.

Qualifications

• Master’s degree in social work.
• State certification and current driver’s license.
• Proof of CPR and Hepatitis profile.

Responsibilities

• Assess home health patients to identify psychosocial, financial, environmental, and community needs as evidenced by POC, documentation and referrals.
• Perform psychosocial assessments, analysis, counseling, and referrals to meet patient/family needs.
• Meet mandatory continuing education requirements and maintain knowledge of eligibility standards.
• Promote agency policies to ensure quality of care and service delivery.
