Job summary

Angels Care Home Health is hiring a Social Worker (MSW) to provide coordinated in-home care across all ages, performing psychosocial assessments, counseling, and referrals.

The MSW will assess patients to identify psychosocial, financial, environmental, and community needs, and will work with the team to meet care goals while maintaining ongoing education and adherence to agency policies.

Qualifications

• Master’s Degree in Social Work.
• State Certification.
• Current State Driver’s License.
• One year of MSW experience in healthcare.
• CPR certification and Hepatitis profile.

Responsibilities

• Assess home health patient to identify psychosocial, financial, environmental, and community needs.
• Use effective interpersonal relations and communication skills with the care team to achieve outcomes.
• Meet mandatory continuing education requirements.
• Promote Agency philosophy and administrative policies to ensure quality of care.
