Prime HealthCare is seeking a Hospital-based Case Manager at Holy Family Medical Center in Des Plaines, IL. Responsibilities include psychosocial assessments, care coordination, and discharge planning within a multidisciplinary team.

The role emphasizes crisis intervention, intake support, and documentation throughout the patient’s stay, on full-time daytime shifts. A Bachelor’s degree and internship licensure requirements apply, with preferred BCLS certification.

Qualificações

• Bachelor’s degree in Social Work, Marriage and Family Therapy or related field required.
• Current registration as an intern with the licensing board as appropriate.
• Behavioral Violence Prevention training within 3 months of hire and maintained annually.
• Minimum one year inpatient or outpatient behavioral health experience preferred.
• Master’s in Psychology, Social Work, or related field; LPCC or equivalent preferred.
• Current BCLS (AHA) certificate upon hire preferred.
• Current BCLS certificate preferred.

Responsabilidades

• Coordinate case management and milieu management, complete psychosocial assessments, assist in treatment plans, and work with a multidisciplinary team.

Overview

Department: Case Management

Schedule: Full-time - Days, 8am-4:30pm

Facility: Holy Family Medical Center

Location: Des Plaines, IL

What we are looking for:

Hospital-based case management or care coordination experience

Strong discharge planning experience with the clinical knowledge to anticipate and address patient care needs

Basic understanding of care transitions and alternate levels of care

Responsibilities

To provide emotional support, crisis intervention and counseling; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability or trauma. To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of a multidisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

Required qualifications:

1. Bachelor’s degree Social Work, Marriage and Family Therapy or other related required.

2. Current registration an intern with Board., as appropriate to education and licensing requirements.

3. Behavior Violence Prevention within 3 months of hire and maintain current annually.

4. Minimum one (1) year Inpatient l or Outpatient Behavioral Health experience preferred.

Preferred qualifications:

1. Current BCLS (AHA) certificate upon hire and maintain current preferred.

2. Masters in Psychology, Social Work, Marriage and Family Therapy, Licensed Professional Clinical Councilor or related field, Preferred.
