Job summary

Aurora St. Luke’s South Shore in Wisconsin is seeking a Medical Social Worker to deliver patient-centered social work services in the hospital setting. You will assess psychosocial needs, develop care plans, and coordinate with the care team to promote positive outcomes.

This role requires an MSW from an accredited school and Wisconsin social work certification, with 1–2 years in a clinical/hospital setting preferred. Full-time schedule, Monday–Friday, with weekend coverage as needed.

We’re Looking For

Licensure or Certifications

• WI locations -Social Work Certification issued by the State of Wisconsin

Education/Experience

• Masters of Social Work services from an accredited school
• Years of Experience: 1-2 years of experience in a clinical, hospital setting post licensure preferred.

As a Medical Social Worker, You Will

• Provide comprehensive social work services, including assessments, care planning, interventions, and discharge coordination, to meet patients’ psychosocial needs.
• Collaborates with multidisciplinary teams to ensure safe, timely care transitions and access to services such as post-acute care, mental health support, and community resources.
• Educates and advocates for patients and families, connects them with appropriate services, and addresses issues like end-of-life planning, abuse, and social needs. Manages care progression to optimize outcomes, reduce readmissions, and ensure appropriate levels of care.
• Maintain documentation, compliance with regulations, and professional standards, while supporting staff training and continuing education.

Schedule: Full time, 40 hours a week, Monday-Friday day shift with weekend rotation.
