Job summary

Aurora BayCare Medical Center in Green Bay, Wisconsin, seeks a Medical Social Worker to provide comprehensive psychosocial assessments, care planning, and discharge coordination for patients across care transitions. You will collaborate with multidisciplinary teams, advocate for patients and families, and help connect them with post-acute services, mental health support, and community resources.

This role requires an MSW from an accredited school, Wisconsin licensure, and 1–2 years of hospital

• Years of Experience: 1-2 years of experience in a clinical, hospital setting post licensure preferred.

Responsibilities

• Provide comprehensive social work services, including assessments, care planning, interventions, and discharge coordination, to meet patients’ psychosocial needs.
• Collaborates with multidisciplinary teams to ensure safe, timely care transitions and access to services such as post-acute care, mental health support, and community resources.
• Educates and advocates for patients and families, connects them with appropriate services, and addresses issues like end-of-life planning, abuse, and social needs. Manages care progression to optimize outcomes, reduce readmissions, and ensure appropriate levels of care.
• Maintain documentation, compliance with regulations, and professional standards, while supporting staff training and continuing education.

• License/Registration/Certification: IL locations-LSW Eligible, LCSW preferred issued by the State of Illinois

What You’ll Do

Schedule: This position is part of a job share, 40 hours over 2 weeks. 7:30am-4:00 pm. Weekend rotation (approximately 6 per year). On-Call rotation (approximately 15 days per year). Holiday rotation (no more than 1 holiday per year) Currently the position is Mon/Tues one week, then Wed/Thurs/Fri the other week.

Physical Requirements And Working Conditions

• This position requires travel, therefore, will be exposed to weather and road conditions.
• Operates all equipment necessary to perform the job.
• Exposed to a normal office environment.
• Exposed to patient care environment. May be exposed to hazardous materials and life-threatening diseases, therefore team members must abide by personal protective equipment as ordered.
• Weekend, holidays, and evening coverage per site requirement.
• Position requires sitting, standing, speaking, reading/writing, and walking throughout the workday.
• Fast paced work environment with established time constraints per site.
