Job summary

Advocate Christ Medical Center in Oak Lawn, IL is seeking a Medical Social Worker to provide comprehensive psychosocial assessments, care planning, and discharge coordination for patients. You will collaborate with multidisciplinary teams to ensure safe, timely transitions and connect patients with post-acute care, mental health support, and community resources.

You will educate families, advocate for patients, support end-of-life planning, and manage documentation to meet regulatory and quality

Qualifications

• Masters of Social Work from an accredited school.
• 1–2 years of experience in a clinical, hospital setting post licensure preferred.
• IL LSW eligible; LCSW preferred (Wisconsin also listed for WI locations).

Advocate Christ Medical Center is a prominent non-profit teaching hospital located in Oak Lawn, Illinois. Licensure or Certifications

• License/Registration/Certification: IL locations-LSW Eligible, LCSW preferred issued by the State of Illinois
• WI locations -Social Work Certification issued by the State of Wisconsin

Education/Experience

As aMedical Social Worker, you will:

• Provide comprehensive social work services, including assessments, care planning, interventions, and discharge coordination, to meet patients’ psychosocial needs.
• Collaborates with multidisciplinary teams to ensure safe, timely care transitions and access to services such as post-acute care, mental health support, and community resources.
• Educates and advocates for patients and families, connects them with appropriate services, and addresses issues like end-of-life planning, abuse, and social needs. Manages care progression to optimize outcomes, reduce readmissions, and ensure appropriate levels of care.
• Maintain documentation, compliance with regulations, and professional standards, while supporting staff training and continuing education.

Schedule:Per Diem, Minimum of two 8-hour shifts per 4-week schedule, at least 1 shift worked must be weekend shift. One Summer Holiday (Memorial Day, 4th of July, Labor Day) and One Winter Holiday (Thanksgiving Day, Christmas Day, New Year's Day) per year
