Job description

**Job Description:**

The Social Work Care Manager collaborates with physicians and healthcare team members to enhance patient health, focusing on those with chronic conditions or complex needs. The role involves educating patients and caregivers, facilitating communication, coordinating services, addressing barriers, and promoting optimal resource allocation, all while balancing clinical quality and cost management.

**Job Specifics:**

  1. Pay Range Clinical: $34.06-$52.55 Non Exempt
  2. Pay Range Certified: $30.94-$47.77 Non Exempt
  3. Benefits Eligible: Yes
  4. FTE: Part time
  5. Shift: Days, 0800-1630, every other weekend
  6. Remote position

**Job Essentials:**

  1. Identify patient populations using screening criteria, including psychosocial screening, medical record review, or referrals, following department guidelines.
  2. Review medical records and relevant reports to gather data; interview patients and families as needed.
  3. Assess patients per CMS COP or NCQA standards and develop appropriate care plans.
  4. Coordinate communication among patients, caregivers, healthcare team, and external parties, ensuring timely and appropriate services.
  5. Intervene to resolve barriers across the care continuum, maintain knowledge of community resources, and perform cost-benefit analyses.
  6. Coordinate authorizations and support mental health diagnoses and interventions as appropriate, acting within legal scope.

**Minimum Qualifications:**

  1. Master's Degree in Social Work from an accredited institution.
  2. Current State Licensure as a CSW or LCSW.
  3. Basic computer skills, including word processing and spreadsheets.
  4. Strong communication and critical thinking skills.

**Preferred Qualifications:**

  1. Case Management Certification.
  2. Experience in clinical care management or with third-party payers.
  3. Knowledge of care management, health insurance products, coding, and care guidelines.
  4. Independent, motivated, adaptable, with positive attitude.

**Physical Requirements:**

  1. Communicate effectively with others.
  2. Operate computers and office equipment.
  3. Read monitors and documents.
  4. Sustain long periods of sitting or standing for work.

**Location:** Valley Center Tower, Murray, Utah

**Scheduled Weekly Hours:** 24

The hourly rate varies with experience, ranging from $34.06 to $52.55.
