Responsibilities:

GENERAL PURPOSE
The social worker provides psychosocial assessment, psychiatric diagnosis, and behavioral health treatment for adult patients 18 years and older in the Adult Ambulatory Clinic. The role includes functioning as a primary therapist, coordinating treatment plans, participating in interdisciplinary teams, developing community liaisons, and providing crisis intervention. The social worker may also lead therapeutic groups and conduct intake assessments.

ESSENTIAL FUNCTIONS

1. Performs screening, diagnostic formulation, evaluation, and psychosocial assessments. Provides social work and behavioral health treatment including individual, family, and group therapy. Coordinates treatment and discharge planning.
2. Works collaboratively as part of the interdisciplinary team. Develops and implements treatment plans, requests psychiatric consultations, and secures treatment authorizations.
3. Coordinates with community resources and provides linkages to ensure continuity of care. Offers backup coverage for colleagues as needed.
4. Documents all treatments in the electronic medical record in accordance with policies.
5. Participates in staff meetings, policy implementation, discharge planning, and quality assurance activities. Contributes to creating a culturally competent environment and engages in cultural competency training.
6. Provides backup support to other units/programs during absences.

Qualifications:

1. Master’s degree in social work from an accredited school (CSWE required).
2. 2-3 years of professional social work experience beyond the Master’s degree, preferably including psychiatric emergency, crisis, or outreach work.
3. Experience in individual or group therapy in an acute setting is preferred.
4. Knowledge of behavioral health assessments, evidenced-based interventions, and a biopsychosocial approach is required.
5. Commitment to team-based, integrated care and cultural humility is essential.
6. Licensure as a social worker by the State Board is required, with a deadline of 18 months to obtain licensure if not already licensed at start, or a limited permit if working under OMH.
