Job summary

Saint Anthony Hospital in Chicago, IL is seeking a Social Worker to plan, organize and deliver social work services for adult psychiatric patients. This is a full-time daytime role with responsibilities centered on discharge planning and patient support.

You will conduct psychosocial assessments, coordinate with medical teams, assist with placement and aftercare, and participate in weekly inpatient group therapy. A Master’s in social work and LCSW preferred are desirable qualifications.

Position Purpose: Plans, organize and implements social work services for adult psychiatric patients.

Essential Functions
  • Conducts psychosocial assessments of patients and family members to obtain information about home environment, family relationships, perception of patient’s illness and coping resources in order to provide emotional support and discharge planning assistance. Documents same in the patient's medical record.
  • Encourages understanding and acceptance of any illness as well as the treatment plan. Along with this he/she gathers pertinent information from medical records, hospital personnel and significant others to formulate a comprehensive discharge and post-hospital plan of care.
  • Communicates patient/family issues to appropriate staff members and attending physicians. Participate in team and/or family conferences to ensure patient understanding and compliance with the medical plan.
  • Collaborates with patients/significant others and/or physicians on discharge planning, such as nursing home placement, halfway house, shelter or other residence placement, rehabilitation placement, home health, transportation, etc. Plans for discharge plans as needed.
  • Assesses and intervenes in abuse/neglect cases (all ages). Prepares and maintains required documentation on each case.
  • Participates in the Advance Directives Program as a liaison to provide patients information on Advance Directives.
  • Leads weekly group therapy session on inpatient unit.
Marginal Functions
  • Performs both essential and marginal job functions in a safe manner as identified by Saint Anthony Hospital and the respective department while adhering to the Hospital’s confidentiality norms and standards. Actively and consistently demonstrates the core values of Saint Anthony Hospital in all interactions with others.
  • Assumes responsibility for knowing and maintaining established hospital and departmental objectives, policies/procedures including general sanitation, safety, environmental and infection control standards.
  • Participates in performance activities including problem identification, data collection, solution selection, implementation and evaluation. Also assists in the care and maintenance of departmental equipment and supplies as appropriate.

  • Master’s in social work, LCSW preferred.
  • Requires excellent observation skills, analytical thinking, problem-solving, and ability to communicate and interact with all levels of professionals, in addition to good verbal and written communication skills
  • Good critical thinking skills; ability to assess, evaluate, and teach. Flexible with a working knowledge of all areas of medicine.
