Job summary

Continuum Hospice & Palliative Care California is seeking a dedicated Medical Social Worker (MSW) to deliver compassionate social work services to hospice patients and families in Marin and Sonoma Counties. The role focuses on counseling, emotional support, and guiding coping strategies within a patient-centered care model.

Responsibilities include psychosocial assessments, care planning with the interdisciplinary team, and collaboration with Bereavement services.

Qualifications

  • MSW from an accredited school of social work.
  • Licensed as a Social Worker from the State Board.
  • Current Driver's License and vehicle insurance.

As a Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Your efforts will make a profound difference in their lives during challenging times.

Responsibilities

Qualifications
Education
  • Required: Master of Social Work from an accredited school of social work
License Requirements
  • Preferred: Licensed as a Social Worker from the State Board of Social Work
  • Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle
Experience
  • Preferred: Minimum one (1) year experience as a social worker in a hospice setting
  • Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred
Additional State-Specific Requirements

CA: Possess and maintains current CPR certification

$51.00 - $55.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
