Job summary

Hospice of the West is seeking a dedicated Licensed Medical Social Worker (MSW) to deliver compassionate social work services to hospice patients and their families in the Phoenix metro area.

You will assess psychosocial status, coordinate with the care team, and provide individualized care planning with a focus on comfort, dignity, and emotional support. The schedule is Monday–Friday, 8 am–5 pm with rotating on-call, offering a strong benefits package and growth opportunities.

Qualifications

  • Master of Social Work from an accredited school of social work
  • Bachelor of Social Work from an accredited school of social work (must be supervised by MSW)
  • CPR certification may be required in some states

Responsibilities

  • Assess and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
  • Evaluate medical needs, home situations, financial resources, and community support.
  • Implement personalized social work plans and collaborate with the Bereavement Coordinator.
  • Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
  • Provide empathetic counseling to patients and families.

Are you a dedicated Licensed Master Social Worker (LMSW) who believes compassionate care can change lives? At Hospice of the West, we’re looking for a patient-centered professional to join our team.

As a Licensed Medical Social Worker (MSW) with us, your primary responsibility will be delivering exceptional social work services to our hospice patients and their families. You'll provide counseling and guidance to support their emotional well-being and coping capacity. Your efforts will make a profound difference in their lives during challenging times.

Schedule: Monday - Friday, 8 am - 5 pm with Rotating On-Call

Coverage Area: Phoenix Metro Area

Responsibilities

Qualifications

Education

  • Required: Master of Social Work from an accredited school of social work
  • Required: Bachelor of Social Work from an accredited school of social work (must be supervised by MSW)

License Requirements

  • Preferred: Licensed as a Social Worker from the State Board of Social Work
  • Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

  • Preferred: Minimum one (1) year experience as a social worker in a hospice setting
  • Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

Additional State-Specific Requirements:

AL, AZ, CA: Possess and maintains current CPR certification

$27.50 - $33.50 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
