**Job Description:**

The Social Work Care Manager collaborates with physicians and healthcare team members to enhance the health outcomes of patients with chronic or complex conditions. The role involves educating patients and caregivers, facilitating communication, coordinating services, addressing barriers, and promoting efficient resource use while balancing clinical quality and cost management.

**Job Specifics**

  • Pay Range Clinical: $34.06-$52.55 Non Exempt
  • Pay Range Certified: $30.94-$47.77 Non Exempt
  • Benefits Eligible: Yes
  • FTE: Part time
  • Shift: Days, 0800-1630, with every other weekend requirement

**This position is fully remote**

**Job Essentials**

  • Identify patient populations using screening criteria, medical record review, or referrals, including trigger lists, timeframes, and pre-screen criteria. Review medical records and relevant reports to gather data. Obtain additional information through patient/family interviews. Assess patients per CMS COP or NCQA standards. Develop and maintain care plans for members receiving care management services.
  • Coordinate communication among members, caregivers, healthcare team, and external parties. Ensure appropriate sequencing of tests, procedures, and interventions. Intervene to resolve barriers across the care continuum. Maintain knowledge of community resources. Perform cost-benefit analyses and coordinate authorizations for care levels. May diagnose mental, emotional, and behavioral disorders per DSM 5 and provide social work interventions, including crisis support. Collaborate with external agencies when abuse, neglect, or exploitation is suspected or confirmed. Act within the scope of the Mental Health Practice Act.

**Minimum Qualifications**

  • Master's Degree in Social Work from an accredited institution; degree verification required.
  • Current State Licensure as a CSW or LCSW.
  • Basic computer skills, including word processing and spreadsheets.
  • Strong written and verbal communication skills.
  • Critical thinking abilities demonstrated.

**Preferred Qualifications**

  • Case Management Certification.
  • Experience in clinical care management, social work, or third-party payers.
  • Knowledge of care management, health insurance products, coding, episodes of care, and length of stay guidelines.
  • Excellent communication skills and ability to work independently, self-motivated, positive, and adaptable.

**Physical Requirements:**

  • Communicate effectively with others.
  • Operate computers and office equipment, requiring finger and hand movements.
  • Read computer monitors and documents.
  • Remain seated or standing for extended periods during work.
