Job summary

Givens Highland Farms in Black Mountain, NC seeks a full-time Health Services Social Worker to provide social services navigation for Skilled Nursing and Assisted Living. You will assess residents, develop care plans, and coordinate with interdisciplinary teams to ensure holistic care and respect residents' rights.

Qualifications

• BSW or related Human Services degree required.
• 2+ years of experience with older adults.
• NC driver's license with clean driving record.

Responsibilities

• Serve as the social services point of contact for Health Care Center and Residential Assisted Living.
• Collaborate with care teams to ensure comprehensive, individualized resident care.
• Complete psychosocial assessments and develop/monitor care plans.
• Uphold Residents’ Rights and document complaints or grievances.
• Coordinate discharge planning and transitions with families and external agencies.
• Assist with admissions and provide backup support as needed.

Givens Highland Farms a leading continuing care retirement community in Black Mountain, is hiring a full-time Health Services Social Worker. The primary purpose of the Health Services Social Work Navigator position is to provide social services navigation and support for both the Health Care Center (Skilled Nursing/Long Term Care) and Residential Assisted Living (Multi-unit Assisted Housing with Services). The Social Worker will have a unique opportunity to channel their knowledge, skills, ambition, and experience in a meaningful way that directly impacts the lives of our residents.

What You'll Do

• Serve as the social services point of contact for the Health Care Center and Residential Assisted Living
• Collaborate with the Health Services Director, Director of Nursing, and interdisciplinary Care Plan Team to ensure comprehensive and individualized resident care and services are provided
• Complete comprehensive psychosocial assessments and develop, implement, and monitor individualized care plans for residents in both the Health Care Center and Residential Assisted Living
• Advocate for and uphold Residents’ Rights, including documentation and follow-up on resident and/or family complaints or grievances
• Coordinate discharge planning, and care transitions with residents, families, interdisciplinary Care Plan Team, and external agenciesAssist with coordinating admissions and serve as backup to the admissions coordinator, as needed
• Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor

What You'll Need

• Bachelor's degree in Social Work (BSW) or a related Human Services Field required
• 2+ years of experience working with older adults
• Must have valid NC Drivers License with clear driving record for use of facility vehicle

Compensation is $28 - $32 per hour, and includes our comprehensive benefits package!
