Job summary

Lifepoint Rehabilitation, part of Lifepoint Health, seeks a Social Worker Coordinator for St. Charles Health System in Bend, Oregon. The role supports inpatient rehabilitation and coordinates patient care across teams, with emphasis on family education and discharge planning.

Be a CSWE-accredited MSW graduate with current licensure; ACSW desirable. A $5,000 sign-on bonus is offered for this position.

Qualifications

• Must have MSW degree from CSWE-accredited program and current license/certification.
• ACSW certification desirable.
• Experience with adults/geriatric populations; disability experience desirable.

Responsibilities

• Participate in pre-admission screening as requested.
• Conduct initial assessments.
• Develop individualized treatment plans.
• Provide age-appropriate therapeutic interventions.
• Evaluate patient and family response to treatment.
• Document the course of patient care including progress and need for continued treatment.
• Establish and maintain therapeutic relationships with patients and families.
• Provide patient and family education on tasks/resources for continued functional independence.
• Obtain pre-certification and re-certification of insurance coverage as appropriate.
• Participate in continuing education/professional development.
• Communicate professionally with patients, families, coworkers and others.
• Support client relations and marketing activities.
• Maintain licenses/certifications and memberships in applicable organizations.

St. Charles Health System is a 13-bed acute rehabilitation unit dedicated to helping patients who are recovering from various injuries or illnesses. In partnership with Lifepoint Rehabilitation, this facility provides 24 hours of nursing care and intensive inpatient therapy to provide an outstanding experience and quality outcomes for all of the patients we serve.

How you'll contribute

A Social Worker Coordinator who excels in this role

• Participate in pre-admission screening as requested
• Conduct initial assessments
• Develop individualized treatment plans

• Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others
• Contribute to client relations and marketing activities
• Maintain competency, licenses/ certifications, and memberships in applicable professional organizations
• Other duties as assigned

What we're looking for

• At a minimum, be a graduate of a CSWE approved master’s degree program and hold applicable and current license/certification to practice
• ACSW certification is desirable
• Education/experience in working with an adult and geriatric population is necessary; education/experience serving people with physical disabilities is desirable
