Job summary

Autumn Lake Healthcare at Spa Creek invites a dedicated Social Worker to join our team and support residents’ emotional and social needs.

The role focuses on directing social services, discharge planning, and coordinating with external agencies, while providing resident-centered information on Medicare/Medicaid and financial assistance options. Candidates should hold an active Social Worker License and bring prior relevant experience.

Qualifications

• Previous experience as a Social Worker preferred.
• Valid Social Worker License.

Responsibilities

• Plan, organize, develop, and direct the Social Services Department to meet residents' emotional and social needs.
• Plan, develop, organize, implement, evaluate and direct the social service programs of this facility.
• Participate in discharge planning, development and implementation of social care plans and resident assessments.
• Interview resident/families as necessary and in a private setting.
• Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
• Provide information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to the resident.
• Additional responsibilities as needed.
