Job summary

The Fargo VA Health Care System is recruiting for 1 full-time Social Worker (Program Coordinator) to serve as Contract Nursing Home Coordinator under the Rehab and Extended Care ICC Service Line. This role requires MSW from CSWE-accredited program, state licensure, and experience in psychosocial assessments and care coordination.

The incumbent will provide clinical social work services, coordinate with VA and community nursing homes, and develop policies for program operation.

Qualifications

• Must have MSW from CSWE-accredited program.
• Must be licensed/certified to independently practice social work at MSW level.
• English language proficiency required.
• Experience with psychosocial assessments and treatment interventions.
• Ability to coordinate with VA and community providers.

Responsibilities

• Perform psychosocial assessments and develop treatment plans.
• Provide clinical case management and advocacy for Veterans.
• Coordinate with VA staff and community resources; serve as liaison for contracted nursing home program.
• Develop and monitor program policies and procedures for the community nursing home program.
• Prepare data, reports, and statistics for facility, VISN, and national leadership.
