Job summary

Collingswood Rehabilitation and Healthcare Center seeks a Social Work Director to lead psychosocial services and advocate for residents and families within our 160-bed Sub-Acute and Long-Term Care facility.

You will guide care planning, coordinate resources, and collaborate with an interdisciplinary team to ensure compassionate, resident-centered support and smooth transitions across care stages.

Qualifications

• Bachelor's degree required; Master's degree strongly preferred.
• Current LSW/CSW licensure required.
• Healthcare social work experience is required.
• Experience in a nursing home or rehabilitation center is preferred.
• Knowledge of regulations and standards governing nursing facilities.

Responsibilities

• Provide leadership and oversight of social services programs for residents and families.
• Ensure services comply with federal, state, and facility guidelines.
• Collaborate with interdisciplinary teams for resident-centered care planning.
• Offer counseling, support, and advocacy during admissions and transitions.
• Act as liaison between residents, families, staff, and external agencies.
• Support staff in addressing psychosocial needs with empathy and professionalism.
• Assist with discharge planning, resource coordination, and community referrals.
• Maintain accurate documentation in line with regulatory requirements.

Responsibilities for Social Work Director

• Provide leadership and oversight of social services programs to support the emotional, psychosocial, and practical needs of residents and their families.
• Ensure services are delivered in accordance with all applicable federal, state, and facility regulations, guidelines, and standards of care.
• Collaborate with interdisciplinary teams to support resident-centered care planning and ensure holistic support for residents’ well-being.
• Provide compassionate counseling, support, and advocacy for residents and families during admission, care transitions, and end-of-life planning.
• Serve as a key liaison between residents, families, staff, and external agencies to ensure effective communication and coordinated care.
• Support and guide staff in addressing resident concerns, behavioral challenges, and psychosocial needs with empathy and professionalism.
• Assist with discharge planning, resource coordination, and community referrals to support smooth transitions of care.
• Maintain accurate documentation and records in compliance with regulatory requirements and facility policies.

Qualifications for Social Work Director

• Bachelor’s degree required; Master’s degree strongly preferred.
• Current LSW/CSW licensure required.
• Experience as a social worker in a healthcare setting is required.
• Experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, standards, and guidelines governing nursing care facilities.
• Excellent communication skills with the ability to interact effectively and compassionately with residents, families, staff, and external agencies.
• Ability to balance strong advocacy with empathy, professionalism, and emotional support.
