Job summary

Yardley Rehabilitation and Healthcare Center is seeking a Social Services Director to provide leadership over psychosocial services for residents and families. You will ensure regulatory compliance, guide care planning, and advocate for compassionate support throughout admission, transitions, and end-of-life planning.

This role requires a strong background in healthcare social work, LSW/CSW licensure, and experience in nursing home or rehab settings.

Qualifications

• Bachelor’s degree is required; Master’s degree strongly preferred.
• Current LSW/CSW licensure required.
• Experience as a social worker in a healthcare setting is required.
• Experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations, standards, and guidelines governing nursing care facilities.
• Excellent communication skills with residents, families, staff, and external agencies.
• Ability to balance advocacy with empathy, professionalism, and emotional support.

Responsibilities

• Provide leadership and oversight of social services programs to support residents and families.
• Ensure services are delivered in accordance with federal, state, and facility regulations, guidelines, and standards of care.
• Collaborate with interdisciplinary teams to support resident-centered care planning and holistic well-being.
• Provide compassionate counseling, support, and advocacy during admission, transitions, and end-of-life planning.
• Serve as liaison between residents, families, staff, and external agencies for coordination of care.
• Support staff in addressing resident concerns, behavioral challenges, and psychosocial needs with empathy.
• Assist with discharge planning, resource coordination, and community referrals.
• Maintain accurate documentation and records in compliance with regulatory requirements.
