Job summary

Yardley Rehabilitation and Healthcare Center in Bucks County, PA, is seeking a Director of Social Services to lead psychosocial services for residents and families. This role provides advocacy, counseling, and coordination across care teams to ensure compassionate, resident-centered support.

• Bachelor’s degree required; Master’s degree strongly preferred.
• Current LSW/CSW licensure required.
• Healthcare social work experience is required.
• Nursing home or rehabilitation center experience preferred.

Responsibilities

Responsibilities for: Social Services Director

• Provide leadership and oversight of social services programs to support the emotional, psychosocial, and practical needs of residents and their families.

• Ensure services are delivered in accordance with all applicable federal, state, and facility regulations, guidelines, and standards of care.

• Collaborate with interdisciplinary teams to support resident-centered care planning and ensure holistic support for residents’ well-being.

• Provide compassionate counseling, support, and advocacy for residents and families during admission, care transitions, and end-of-life planning.

• Serve as a key liaison between residents, families, staff, and external agencies to ensure effective communication and coordinated care.

• Support and guide staff in addressing resident concerns, behavioral challenges, and psychosocial needs with empathy and professionalism.

• Assist with discharge planning, resource coordination, and community referrals to support smooth transitions of care.

• Maintain accurate documentation and records in compliance with regulatory requirements and facility policies.

Qualifications for Social Services Director

• Bachelor’s degree required; Master’s degree strongly preferred.

• Current LSW/CSW licensure required.

• Experience as a social worker in a healthcare setting is required.

• Experience in a nursing home or rehabilitation center strongly preferred.

• Strong knowledge of regulations, standards, and guidelines governing nursing care facilities.

• Excellent communication skills with the ability to interact effectively and compassionately with residents, families, staff, and external agencies.

• Ability to balance strong advocacy with empathy, professionalism, and emotional support.
