• Plans, directs, coordinates, and evaluates Social Work and Activities Programming
• Manages the overall operations of the Social Work Department
• Supervises and coordinates Activities Coordinators and Assistants
• Ensures staff are trained on regulatory guidelines
• Coordinates employee training
• Provides psychosocial assessment of illness-related factors
• Identifies high-risk indicators affecting resident health
• Implements treatment plans and counsels residents/families
• Discharges planning liaison between residents/families and treatment team
• Documents assessments, progress, and notes in medical records
• Identifies community resources and maintains relationships with providers
• Lead admission/discharge planning and orientation for new residents/families
• Comply with regulator and accrediting agency requirements
• Adhere to NMHS policies and procedures
