Qualifications

• Two years hospital case management or discharge planning experience.
• MSW required; LCSW preferred or willingness to obtain.
• BLS certification required; Central Registry required.
• Strong communication and interpersonal skills.

Responsibilities

• Assess patients within an identified caseload prior to admission or on first contact; identify support systems and resources.
• Develop and revise care management plans to optimize outcomes and cost.
• Collaborate with physicians and care team; adjust plans as patient condition changes.
• Identify, implement and document discharge plans; ensure smooth transitions between care settings.
• Provide crisis intervention and emotional support to patients and families.
• Coordinate patient/family physician conferences and education for staff.
