Job summary

Sheridan Memorial Hospital in northern Wyoming invites applications for a Social Work Case Manager. Under general supervision, the role assesses psychosocial and discharge planning needs, provides crisis intervention and coordinates care with the interdisciplinary team.

Requirements include a Master of Social Work, Licensed Clinical Social Worker or willingness to obtain, BLS, and hospital case management experience.

Qualifications

• Masters in Social Work is required.
• Licensed Clinical Social Worker credential is required or willingness to obtain.
• BLS certification is required.
• Central Registry is required.

Responsibilities

• Assesses each patient within an identified caseload prior to admission or on first contact, identifying needs and resources.
• Develops and revises case management plans to optimize cost/quality outcomes.
• Collaborates with physicians and the care team to evaluate progress and adjust plans.
• Identifies and documents discharge planning needs and ensures smooth transitions between care settings.
