Job summary

Allegheny Health Network is seeking a Social Work Case Manager to join the Care Management team in Allegheny Valley Hospital. The role focuses on coordinating care across inpatient and post-acute settings, addressing social determinants of health to improve patient outcomes, and advocating for patients and families.

The position emphasizes collaboration with clinicians, health plans, and community resources to ensure safe transitions and appropriate post-discharge support.

Responsibilities

• Contributes to and/or completes initial and ongoing comprehensive assessment.
• Provides interventions and implements recommendations after engaging patients and families.
• Focuses on social determinants of health to coordinate care across the continuum.
• Collaborates to design the safest transition plan for discharge and post-discharge resources.
• Educates patient and family on level of care post discharge and access to community support.
• Advocates for patient and family through teamwork, payers and post-acute partners.
• Provides counseling on treatment decisions and end-of-life issues.
• Drives goals-of-care discussions and crisis interventions as needed.
• Supports professional growth through certification, mentoring and committees.
• Supports department goals and performs other duties as assigned.
