Job Summary

This position is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients.

Essential Functions

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.
• Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and specialized safety assessment when violence is suspected Plan discharge from care facility to home or other care facility.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
• Knowledgeable in therapeutic behavioral interventions, replacement skill-building strategies, crisis assessment, management, and interventions.
• Strong knowledge of HIPPA, Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations.
• Completes required documentation and paperwork including biopsychosocial assessments and treatment planning and implementation in a timely manner.
• Ability to communicate and resolve issues effectively with a broad population of patients, staff and physicians.
• Ability to work as part of a multidisciplinary team with a varied population of clients.
• Display strong communication skills to clearly guide peers.

• Completed LICSW/LCSW requirements.
• Leads department/service initiatives; participates in initiatives at an Enterprise and Professional Association level.
• Uses data to define baseline issues and measure progress.
• Oversees collaboration and education among all disciplines and outside stakeholders to enhance patient care.
