Ashtabula Regional Medical Center seeks a qualified candidate to coordinate the care and services for patient populations. The role requires knowledge of biopsychosocial factors and crisis intervention skills.
The ideal candidate has a BA in social work or related field, must be licensed as LSW or LISW by the State of Ohio, and experience in healthcare settings. Competitive salary and a comprehensive benefits package are offered.
Demonstrates knowledge of biopsychosocial factors related to the ages of the patient’s served and incorporates knowledge base when developing treatment plan. Coordinates the care and services of patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; assumes a leadership role within the interdisciplinary team to achieve optimal clinical and resource outcomes; facilitates and coordinates the discharge planning process.
BA in social work or related field and LSW. Master degree in Social Work from accredited school or university preferred. Must be able to work independently and as part of a multi-disciplinary team. Crisis Intervention skills, Therapeutic communication skills.
Two years of previous experience in a hospital or healthcare setting.
LSW OR LISW by State of Ohio
Required to walk, stand, bend and sit. Routine and/or daily tasks or procedures occasionally expose them to blood and other potentially infectious materials. Must be able to remain calm and function clearly in a stressful potentially violent environment.
