Job summary

IHA in Columbus, OH is seeking a full-time Social Worker (LSW) to support discharge planning in acute or ambulatory settings. The selected candidate will collaborate with care teams to coordinate transitions of care and ensure timely, safe discharges.

Requirements include a Bachelor degree in Social Work, an active Ohio LSW license, and at least 1 year of discharge planning experience. Strong communication and organizational skills are essential.

• Bachelor degree in Social Work is required.

• Education: Bachelor degree in Social Work
