The community you’ll be caring for:
The role you’ll contribute: The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources, and community assistance programs, and/or require help with transitions of care or discharge planning. The role includes crisis intervention, developing discharge plans, and coordinating care with interdisciplinary teams. Responsibilities include patient evaluations, development and implementation of transition plans, ensuring smooth patient flow, and providing education on resource utilization. The Social Worker operates under supervision of the Care Management Supervisor or Manager and collaborates with the care team to ensure patient-centered, efficient, and cost-effective care.
