The community you’ll be caring for:

The role you’ll contribute: The Social Work Care Manager intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs, funding sources, and qualify for community assistance from a variety of special assistance programs and agencies. They also assist with transitions of care or discharge planning. Additionally, crisis intervention is provided to patients and families with psychosocial needs, and a discharge plan of care is developed for high-risk patient populations. The role receives referrals from interdisciplinary team members, including physicians, RN Care Managers, staff nurses, and others.

• Psychosocial Assessment and Interventions
• Receives referrals for psychosocial complex needs from the healthcare team.
• Provides assessment and reporting interventions in cases of child abuse/neglect, domestic violence, adult/elderly abuse, child protection, sexual assault, and human trafficking as appropriate.
• Provides consult services for patients who may lack decision-making capacity, following guardianship policies and procedures, and coordinating with Care Management leadership.
• Provides consult services for foster care and adoptions.

*Terms: 12-month Employment Contract Required for Relocation

Must live 50 miles outside of campus address.
