Qualifications

• Master’s degree in social work (MSW) is required
• Social Work License valid in Alabama
• Minimum 1 year of experience in social work or related field
• Experience in long-term care or healthcare setting, especially in social services
• Strong understanding of case management and care coordination
• Excellent communication and interpersonal skills
• Ability to work compassionately with a diverse patient population

Responsibilities

• Conduct patient assessments to identify social service needs
• Develop and implement personalized care plans with an inter-disciplinary team
• Provide emotional support and counseling to patients and families
• Advocate for patient needs within the facility and coordinate with external resources
• Maintain accurate and confidential patient records
• Oversee discharge planning and ensure continuity of care post-discharge
