Job summary

Carease Health in Pittsburgh seeks a compassionate Mental Health Provider to join our Collaborative Care team. You will provide emotional support, brief therapy, and mental health assessments to residents in skilled nursing facilities.

Visit designated facilities, conduct assessments, implement short-term therapy, document notes, and collaborate with psychiatric professionals, including a monthly meeting with the psychiatric NP.

Qualifications

• Associates, Bachelor's or Master's degree in a mental health field.
• Knowledge of geriatric mental health and crisis management in long-term care.
• Experience: 1 year in a mental health setting.

Responsibilities

Key Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

Qualifications

• Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree
