Job summary

Carease Health is seeking a part-time Social Work/Care Companion to support residents in skilled nursing facilities in Erie, PA. You will visit facilities, assess needs, and provide short-term, solution-focused therapy and supportive interventions to address immediate concerns.

You will document visits, collaborate with psychiatric professionals, and participate in monthly meetings with the psychiatric NP. The role emphasizes compassionate care and flexible days with paid training and ongoing

Qualifications

• Associates, Bachelor's or Master's in Social Work or related field.
• Experience in geriatric mental health and long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• 1 year of experience.

Responsibilities

About the Role:

As a part-time Social Work/Care Companion, you will visit designated facilities, conduct assessments, and implement short-term, solution-focused therapy and supportive interventions to address immediate concerns. Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
• Education: Associate Degree
• Experience: 1 year
