Job summary

Carease Health in Pittsburgh is seeking a compassionate Mental Health Provider to join our Collaborative Care team, delivering emotional support, brief therapy, and mental health assessments to residents in skilled nursing facilities.

You’ll work with geriatric residents, coordinate with psychiatric professionals, and participate in monthly meetings with the psychiatric NP, with a flexible schedule and paid training.

Qualifications

• Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.

Responsibilities

• Visit designated facilities and provide emotional support and brief therapy.
• Conduct assessments to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain records of interactions and interventions.

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

Qualifications
