Job summary

Dignity Health Connected Living in Shasta County, CA, is seeking a compassionate Social Worker to advocate for individuals and families facing complex challenges. You will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources while collaborating with multidisciplinary teams to promote well-being.

The role requires empathy, strong interpersonal skills, and knowledge of community resources.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology or sociology.
• At least 1 year experience working with the elderly and individuals with disabilities.

Responsibilities

• Conduct in-depth psychosocial assessments and reassessments.
• Refer clients to specialists for evaluations and recommendations.
• Develop and implement care plans and conduct follow-up.
• Identify and develop support systems for clients.
• Consult with health practitioners for integrated care.
• Perform case recording and reporting.

Job Summary and Responsibilities

As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges.

Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well-being.

To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.

• Conducts in-depth assessments/reassessments covering psycho-social, rehabilitation and environmental concerns
• Refers clients to special consultants to provide detailed evaluations and recommendations to improve the client's functional level
• Develops the care plan, implement the services and conduct follow-up and monitoring of the plan
• Identifies and develops support systems for the client
• Consults with the Health Practitioner
• Performs case recording and reporting

Job Requirements

Required

• Bachelors Other in social work, psychology, counseling, rehabilitation, gerontology or sociology
• One (1) year experience working with the elderly and individuals with disabilities

Pay Range

$28.10 - $41.80 /hour
