Position Overview

SOCIAL WORK ASSISTANT (CASE PLANNER)

University Behavioral Associates (UBA) at Montefiore Medical Center in Bronx, New York, is seeking a dedicated and organized individual with a bachelor’s or master’s degree and experience in social services to serve as a case planner for the Family Treatment & Rehabilitation specialized preventive program funded by the NYC Administration for Children’s Services (ACS).

Job Description

This community-based, family-centered program works closely with parents, children, and providers (including ACS) to support families focusing on child safety and well-being. Responsibilities include engaging with families through home and school visits, case management, conducting assessments, making referrals, monitoring treatment engagement, and collaborating with community agencies. The role requires coordination between families, providers, and the legal system, documentation, and participation in supervision. Evening and/or weekend availability may be required.

Qualifications

• Experience with child welfare, mental health, and/or substance use concerns.
• Experience working with families and the legal system.
• Excellent organizational and communication skills.
• Spanish-speaking skills are strongly preferred.

Additional Details

Salary: $32.18/hour. Full-time, regular employment at Montefiore Medical Center, Bronx. Shift: Day, 8:30 AM - 5 PM.
