The Social Services Manager is responsible for the coordination and delivery of social services within the facility. This is a hands-on leadership role that not only oversees the Social Services Department but also provides direct services to residents and families. The Social Services Manager ensures that high-quality care and support are consistently provided while supervising one Social Services Case Aide and working collaboratively with the interdisciplinary team.

The responsibilities of the Social Services Manager include, but are not limited to:

Responsibilities:

  • Provide direct social services to residents, including assessments, support planning, and ongoing counseling, while also overseeing the daily operations of the Social Services Department.
  • Supervise, support, and evaluate the performance of one Social Services Case Aide.
  • Assume authority, responsibility, and accountability for directing all aspects of the social services program.
  • Ensure all clinical social services, including supervision of staff, are delivered in accordance with the facility’s Policy and Procedures and as required by Title 22.
  • Plan, develop, implement, and evaluate the facility’s social services programs to meet resident needs and regulatory standards.
  • Complete a social services assessment for each new admission within five (5) days of arrival. Ensure participation in the Interdisciplinary Team (IDT) and completion of the MDS portion within five (5) days, and subsequently on the 14th, 30th, 60th, and 90th days post-admission.
  • Complete additional social service assessments, progress notes, and Person-Centered Plan (PCP) entries for all new admissions within seven (7) days.
  • Ensure MDS documentation and IDT participation occur within fourteen (14) days of admission; complete any required updates within twenty-one (21) days.
  • Educate staff and ensure that all social services documentation in medical records is accurate, descriptive, and reflective of services provided and the resident’s response to those services.
  • Conduct interviews with residents and their families to obtain social histories. Engage residents and families in setting care objectives and individualized programming goals. Facilitate resident and family participation in care planning and coordinate interdisciplinary care conferences as needed.
  • Identify and address social services deficiencies noted during surveys or consultant visits. Develop and implement corrective action plans and provide written documentation of such plans to the Administrator.
  • Participate in facility committees (e.g., Infection Control, Care Plan, IDT, Safety, Room Moves) as assigned by the Administrator. Implement and follow through on committee recommendations.
  • Perform other related duties as required.

Experience and Skills:

  • Education: Bachelor’s degree in Social Work or a related field is required. Master of Social Work (MSW) preferred.
  • Experience: Minimum of two (2) years of managerial experience in a healthcare or long-term care setting.
  • Experience in a Skilled Nursing Facility (SNF) is required.
  • Experience in dementia, memory care, or Alzheimer’s care is preferred.
  • Strong knowledge of federal and state regulations for long-term care facilities.
  • Excellent interpersonal, organizational, and documentation skills.
