Job summary

Signature HealthCARE in Bowling Green, KY, is seeking a Social Services Director for a part-time weekend schedule to support residents’ social, emotional, and psychological needs. You will lead discharge planning, care coordination, and family communications within a 176-bed facility offering short-term rehab and long-term care.

Qualified candidates hold a BA/MA in Social Work or Psychology with 3+ years in social services, preferably in healthcare, and demonstrate confidentiality,

Qualifications

• 3+ years of experience in social services, healthcare setting preferred.
• Bachelor’s or Master's in Social Work or Psychology.
• High level of professionalism and maintaining confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.
