Signature HealthCARE is seeking a Social Services Director for our South Louisville facility. You will identify and address residents' social, emotional, and psychological needs and help plan for discharge to maximize quality of life during the stay.

In this role you will support residents and families with social issues, ensure timely documentation for state and federal compliance, and participate in care and discharge planning. A bachelor's or master's in social work or psychology is required.

Qualifications

• Bachelor’s or Masters Degree in Social Work or Psychology.
• 3+ Years Experience Working in Social Services, directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsabilités

• Assist residents and their families with social, emotional, and family issues.
• Provide timely and accurate completion of documentation needed to comply with Facility, State, and Federal policies.
• Participate in resident care planning.
• Participate in the development of Discharge planning and prepare resident for the transition.
• Serve as a general source of information for residents and their families.
