Position Summary

The Social Services Director is responsible for planning, developing, organizing, implementing, and directing the Social Services Department in accordance with federal, state, and local regulations. This role ensures that residents' psychosocial, emotional, and discharge planning needs are met while promoting resident rights, dignity, and quality of life.

Qualifications

• Required: Bachelor's Degree in Social Work (BSW) or Bachelor's Degree in a related human services field such as Sociology, Psychology, Rehabilitation Counseling, or Gerontology
• Experience in healthcare social services, skilled nursing, long-term care, hospital, hospice, or related healthcare setting
• Must meet all applicable California state and federal qualifications for social services personnel in skilled nursing facilities
• Must meet California Title 22 Social Services Director requirements for a 120-bed skilled nursing facility
• Strong knowledge of discharge planning, care coordination, and resident advocacy
• Excellent communication and organizational skills

Preferred

• Skilled Nursing Facility (SNF) experience
• Previous leadership or supervisory experience in social services
• California social work licensure, when applicable

Schedule Full-Time Monday-Friday Occasional flexibility based on resident and facility needs
