Arbor Hills Nursing and Rehabilitation seeks a compassionate Social Services Director to lead discharge planning and psychosocial care for residents. You will coordinate multidisciplinary teams, assess residents, and promote rights and safety within a large care network.

Qualifications

• Bachelor’s degree in social work or related field is required.
• Current Clinical Social Work Licensure is required.
• At least 1 year of supervised social work experience in healthcare preferred.
• Strong communication with residents and families at all organizational levels.
• Ability to react decisively in emergencies and maintain confidentiality.
• Proficiency in reading, writing, and speaking English.
• Uphold ethical practice and demonstrate relationship management.

Responsibilities

• Ensure social service and discharge planning are integrated across the hospital system via effective communication.
• Develop specific, measurable plans and objectives to improve daily social service operations.
• Perform psychosocial assessments and reassessments for all residents and address needs of residents and families.
• Provide care in a way that maximizes safety and reduces healthcare errors.
• Create teaching plans based on learning needs and involve family in teaching.
• Promote residents’ rights and clinical social work knowledge in practice.
• Interact with residents and families professionally and include them in the care plan.
• Offer individual and group support during adjustment, crisis, or need.
• Direct and coordinate Social Service/Discharge Planning to meet resident needs and comply with policy.
• Provide resources and support to social/service staff and clinical staff.
• Remain flexible, organized, and capable under stress.
• Maintain rapport with medical staff and departments and operate equipment safely.
• Respect and dignify patients and families; uphold safety and quality culture.

Essential Functions of Social Services Director:

Assure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels

• Develop specific, measurable, realistic plans and objectives, which enhance social service dail
• Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their famil
• Perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors
• Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning.Include family in teaching as appropriate.
• Demonstrate knowledge of clinical social work and discharge planning.Interpret and promote residents’ rights and the “Residents’ Bill of Rights”
• Interact professionally with resident and family and involve resident and family in the formation of the plan of care.
• Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.
• Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and facility policy is followed.
• Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.
• Be flexible, organized, and function under stressful situations.
• Maintain a good rapport and working relationship with medical staff, departments, and employees.
• Manage and operate equipment safely and correctly.
• Treat patients and their families with respect and dignity.
• Support and maintain a culture of safety and quality.

Qualifications of Social Services Director

• Bachelor’s degree in social work or a bachelor's degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology
• One year of supervised social work experience in a health care setting working directly with individuals is preferred
• Current State Clinical Social Work Licensure
• Candidates must maintain a working email address and phone number for employer communication.
• Ability to work cooperatively as a member of a team
• Ability to communicate effectively with residents and their family members, and at all levels of the organization
• Ability to react decisively and quickly in emergency situations
• Ability to maintain confidentiality
• Ability to read, write and speak the English language
