Job summary

Middleburg Rehabilitation and Nursing Center is seeking a Social Services Director to oversee the Social Services Department, guide assessments and care planning, and advocate for residents' psychosocial well-being. You will ensure compliance with state and federal regulations and manage documentation and QA processes.

The role includes coordinating discharge planning, facilitating access to community resources, and leading policy development to improve care quality.

Qualifications

• Bachelor's degree in social work or related field required.
• One year of supervised social work experience in a health care setting.

Responsibilities

• Direct the Social Services department, overseeing preadmission services and interdisciplinary evaluation of residents' needs.
• Coordinate discharge planning and arrange community-based services and follow-up care.
• Ensure social services actions are documented in medical records and uphold professional standards.
• Oversee departmental QA procedures and modify them as needed.
• Oversee Advance Care Planning and ensure directives are reviewed regularly with residents or representatives.
• Assist residents in voicing resolutions to grievances; track and report findings for the facility's QAPI program.
• Participate in facility policy development to positively impact quality of care.
• Advocate for residents' rights and dignity; respond to allegations of abuse with appropriate actions.
• Ensure residents with mental illness or psychosocial difficulties have access to appropriate treatment and resources.
