Qualifications

• Meets the requirements for the Social Services Director for a skilled nursing facility or active social worker license in the state of the facility.
• Long term care experience preferred or willing to learn.
• Resident advocate within the structure of communal residential living.
• BS/LSW/MSW required.

Responsibilities

• Coordinates the social services department to promote the psychosocial, spiritual, and emotional well-being of residents, families and staff.
• Oversees the daily functioning of the Social Service Designee.
• Provides Medicaid-related follow-up and assists with Medicaid approval/reapplication processes.
• Discharge planning coordination with follow-up community services, home health referrals and alternative placements.
