Rose Mountain Care Center in New Brunswick, NJ is seeking a Full Time licensed Social Services Director with experience to lead our social services program and ensure regulatory compliance across NASW guidelines. This role requires CSW/LSW certification and knowledge of OBRA/DOH regulations.

Formación

• Graduate with a social work or related degree; SW BA preferred.
• State certification as CSW/LSW required.
• Experience with MDS 3.0, PHQ-9, BIMs in long-term/sub-acute care preferred.

Responsabilidades

• Perform assessments: psychosocial status, cognitive status, adjustment needs.
• Complete documentation: initial history within 14 days; MDS/CAA; IDCP; care plan updates; annual notes; 48-hour hospital updates.
• Discharge planning; family contact; resident/family group facilitation.
• Develop and review care plans; ensure MDS components completed and signed.
• Participate in interdisciplinary meetings; ensure quality customer service.
